Scientific rationale

The sleeve gastrectomy has been the world’s most successful surgery in the field of barytric surgery for many years and represents alone about the 60% of all bariatricprocedures performed every year. Despite the widespread spread of the procedure, we are still far from achieving a standard shared from the point of view of surgical technique. Literature data shows that the rate of complications, effectiveness, comorbidity resolution and gastroesophageal reflux varies greatly in different centers and latitudes in the world. This reflects the fact that, in addition to the problem of follow-up – fundamental to achieve good results in the medium-long period – probably when we say "sleeve gastrectomy" we often refer to an ideal procedure that then, in practice, can be freely interpreted in some passages. Parallel to the bariatric surgery, in recent years the bariatric endoscopy has also developed. Endoscopy Sleep Gastroplasty its main expression. The two options are not to be considered as "competition", but rather as the possibility of offering a personalized treatment based on the different factors in play. In this context, collaboration and cooperation is fundamental, possibly within paths shared by a multidisciplinary team. This meeting of experts wants to compare different operational approaches, endoscopic surgery, to stimulate a discussion that can lead to the acquisition of greater certainty and, if possible, try to approach the definition of "standard procedure". In addition, a significant space will be devoted to endoscopic treatment of complications, a true life-saving for our patients who, unfortunately, should develop the fistolous complication in the post-operative period.
